Day of Appointment
Please be prepared to provide the following:

• Your current insurance information
• Photo ID (Driver's license, passport, etc.)
• List of all prescriptions and non prescription
  (vitamins,herbal, over-the-counter) medications
• List of medication allergies
• Complete medical history and patient

Emergency Contact Information
We can be reached for after hour emergencies by calling our office at (503) 229.7137 and the answering service will contact our on call physician.

All other contacts including test results, billing information, prescriptions or refills can be made during regular business hours, 8:30 am to 5:00 pm.
